Revoli Construction was cited for seven willful violations, 33 repeat violations and 17 serious violations after fatal Yarmouth, Mass., cave-in, according to the U.S. Dept. of Labor.

Worker killed at South Yarmouth force main construction site is identified as Miguel Reis, 61, from Fall River, Mass.Two others escaped the Nov. 18 accident. 

Hurtado Construction Co. has a history of past OSHA citations and officials said another of its employees was killed in a trench accident in 2007.

OSHA found workers were exposed to another potential collapse in the same trench a month after the fatal incident.

Authorities are probing a trench collapse that killed two construction workers, who were identified by The Port Authority of New York and New Jersey as Francisco Reyes, 41, and Fernando Lagunas Pereira, 28.

There were 22 trench-related deaths in the first half of this year, up from 15 in all of 2021.
